We welcome an experienced Care Assessment Coordinator to join the team and work across two of our sites in Hallam and Frankston North.

 

Homewood (Hallam)
Located in the south-eastern Melbourne suburb of Hallam, Homewood is a 180 bed modern Home. Homewood offers permanent and respite aged care accommodation within a very short distance from public transport. 

Forest Lodge (Frankston North)

Located in the Melbourne bayside suburb of Frankston North, Forest Lodge is a 180 bed Home in a quiet suburban setting.  Forest Lodge is bright and airy, providing high-calibre care in a contemporary setting. 

 

The Care Assessment Coordinator will support the monitoring and auditing processes to ensure compliance, monitoring of assessments and care planning practices to maximise funding within the Aged Care Accreditation Standards.

 

What you’ll be doing

  • Demonstrate a high-level understanding of AN-ACC funding processes, including preparation and management of required documentation
  • Review clinical documentation to maximise AN-ACC funding
  • Ensure compliance standards are met and objectives achieved
  • Provide recommendations on development opportunities, improvements and uplifts
  • Provide ongoing AN-ACC support and education to staff

What you’ll need 

  • Certificate III Individual Support (Ageing)  
  • Understanding of the Aged Care Quality Standards   
  • Up-to-date COVID-19 vaccinations (3 doses)  
  • 2024 influenza vaccination  
  • NDIS Check (includes Police Clearance)  
  • Eligibility to work in Australia
